Laser sights are gun attachments which shine a red-colored laser beam to assist accuracy. They are seen many times throughout the Madness Combat series. The first one appeared on Hank's M1911 and on several Desert Eagles in Madness Apotheosis. It appeared again on Hank's suppressed M1911 in Madness Depredation. All of the MK-23 pistols in the series have laser guides. An activated laser sight appeared on the modified FNC from Madness Inundation. Sanford also had a laser sight on his MP5 in ROMP.FLA.

In Madness Interactive, a cheat can be used to give all of the player's guns laser sights. They also appear on various firearms in Madness: Project Nexus (Classic), which increase the range of the weapon by 20%. So far, all playable guns in Madness: Project Nexus are compatible with laser sights.
